What you will do in your working day
- To assist with all aspects of care for the customer in line with their care plans
- To assist in updating care plans through communicating changes to the office team as required
- To write daily notes and record care details
- To raise any concerns with the care manager
- To undertake and complete all relevant training
- To communicate effectively with customers, their families and others involved in care delivery with the support of the care manager
- To be an ambassador for the company
- It is important to be a caring person with good customer service skills and good levels of written and verbal communication skills
- Ability to perform tasks as detailed on the care plans and via the care management system
- To be proactive and to pay attention to details and customer needs
- Team player who is self-motivated
- Ability to input to and maintain records
