To respond to Discharge to Assess and complete initial assessment.
To provide intensive personal, practical and emotional support by promoting independence and choice to people with disabilities, older people or ill people. This will involve helping them to work towards their outcome – focused reablement and rehabilitation goals.
To support the Reablement Lead in the day-to-day supervision of the enablers and to monitor staff performance.
To assist with the induction and on the job training for new staff members. Drivers License is required. YH will accept candidates who are in the process of obtaining the license.
