    Working hours will be Monday to Friday between 7am - 9pm on shift rota base. (8 hours a day). Will need to work some weekends. Additional pay incentive will be added for weekend working.

Main purpose of the post:

To enable service users to lead their own lives as independently as possible and to develop and maintain relationships with those significant to them and with others in the community. To work with other staff, healthcare professionals and agencies to meet the needs and aspirations of service users. To improve the quality of their lives, their involvement in the community and to contribute to the development of a warm and friendly home environment; promoting individual dignity and respect.

Main duties and responsibilities:

  • To use a person centred approach in delivering care, service user support and co-ordination of work with multi disciplinary teams
  • To work in partnership with service users to develop their independence and to achieve their personal goals by supporting development of skills for independence
  • To support service users in developing good relationships with family and friends and people from different walks of life
  • To encourage service users to experience new things and be able to enjoy activities which are part of everyday life
  • To develop links between service users and the local community if appropriate
  • To provide direct care and support, including personal care to service users to enable them to live as independently as possible. This will include supporting service users in undertaking domestic duties, maintaining personal hygiene, accessing educational opportunities and accompanying service users to appointments, shopping, outings,   holidays etc.
  • Prompt and/or administer medication in accordance with First Practice Healthcare Ltd policy and procedures after appropriate training
  • To help service users express themselves through choice and self-advocacy and to protect their rights and individuality
  • To support service users in meeting their cultural and religious needs
  • To liaise with families and friends of service users as required
  • To act as key worker to one or more service user
  • To monitor service users progress and maintain accurate records
  • To support service users in contributing to the domestic routine of their homes and to undertake domestic duties as required ensuring the health, safety and well being of service users
  • To help service users manage their health care arrangements including attendance at appointments, arranging health care screening and seeking advice from medical professionals as required
  • To act as responsible person taking lead responsibility for the health, safety and well being of service users and staff for the duration of the shift, including undertaking lone night work where appropriate
  • To give support in provision of services that meet the service users physical, cultural, emotional, spiritual and educational needs within the home and/or in conjunction with other providers
  • To manage and maintain services in accordance with the principles and practice of equality, taking into account of individual needs and requirements

Quality of service:

  • To act at all times in accordance with First Practice Healthcare Ltd code of conduct and CQC
  • To report immediately to the office staff and/or manager any incident or practice that may be to the detriment of service users in accordance with First Practice Healthcare Ltd Policy
  • To work within the Service Quality Framework to deliver person centred services that meet identified needs and to participate in quality audits
  • To work at all times in accordance with First Practice Healthcare Ltd Policy and comply with procedures
  • To work in accordance with the Care Standards Act 2000

Health & Safety:

  • To assist in assessing risks and preventing and reporting incidents to service users within and outside the home
  • To comply with all health and safety rules and legislation applicable to the service users home, for example, food hygiene, manual handling and fire safety
  • Employees must be aware of the responsibilities placed on them under health & safety legislation to ensure agreed safety procedures are carried out to maintain a safe environment for service users, other employees and visitors

Administration:

  • To maintain accurate paper records in accordance with First Practice Healthcare Ltd policy and procedures, including service users files, accident and incident records, medication records and visit report sheets 

Staff Training and Supervision:

You will be required to:

  • Undertake, in accordance with First Practice Healthcare Ltd policy, an induction and foundation shadowing training
  • Be employed for a probationary period of 3 months
  • Be shadowed and spot checked by a supervisor on commencing the provision of care in the community. The purpose of this is to monitor care worker competence and to ensure that care workers are comfortable with company procedures
  • Enrol on a NVQ level 2 care course if not already on one and complete it within 3 years
  • Attend regular supervision sessions with the manager and to participate in the performance appraisal process
  • Attend specific training sessions relating to Health and Safety and Moving & Handling in order to ensure and maintain a safe working environment
  • Attend ongoing training sessions, which will include training specific to the service user you will be caring for and/or supporting

Future prospects

Can lead to higher level apprenticeship or full-time employment after successfully completing apprenticeship.

Things to consider

Due to the nature of the role, you must be 18+ years old. 

Successfull candidate will have a Reference and DBS Check done before start of employment. 

The locations of the roles will be based around the areas, B28, B29, B31, B32, B45.
