  • To undertake all duties that are outlined in the service users care plan

  • To prepare and cook meals in the Service Users home

About the employer

We provide outcome-focussed, strengths-based and person-centred support designed to suit individual customer needs. Each customer will have a Support Plan in place which will be devised by us in partnership with them, their families and circle of support.

We use a robust approach to the assessment of needs, using the information on the referral form initially as the basis for this. Customers and their circle of support will be included in the process to develop and finalise the support plan. This plan will be kept under regular review to ensure that it continues to meet the changing needs of the customer. The referral, assessment and planning will be conducted under the oversight of the service manager in order to provide continuity.
