Your Healthcare Community Interest Company’s Reablement service provides a Domiciliary Home Care and Reablement Service. The service provided short term planned, intensive intervention to maximise independence following a period of illness, disability or loss of self-confidence.

This is an exciting opportunity for a highly motivated Apprentice to join our team at Surbiton Health Centre. You will provide reablement and rehabilitation support as the service user develops their own independence.

This includes:

  • Giving guidance and direction to service users and their carers in respect of daily living tasks, for example, dressing and personal care, meal preparation and practical support in accordance with the support plan
  • You will provide intensive personal, practical, social and emotional support by promoting independence and choice to people with disabilities, older people or ill people
  • This role will involve helping them to work towards their outcome-focused reablement and rehabilitation goals as set out in their support plan

In addition to meeting the person’s needs this role will:

  • Reduce the need for hospital or residential admission
  • Facilitate a safe and timely discharge
  • Provide active recovery and reablement/rehabilitation, thus preventing unnecessary loss of independence

Requirements and prospects

Desired skills

  • A good level of written and verbal communication skills
  • Good knowledge of IT software to include Word and Excel

Personal qualities

  • A self-starter who is keen to learn
  • Punctual
  • The ability to work as part of a team in a professional environment
  • Organised

Desired qualifications

4 x GCSE A*-D/9-3 including maths and English or equivalent or relevant work experience and required initial assessment results.

Future prospects

Upon completion the apprentice will be encouraged to further progress their career within the organisation.

Things to consider

Candidates are required to have an enhanced DBS and provide the result prior to starting.

Ideally need to be 18+ with a driving licence and a car (this is preferred but not essential).
