Your Role at Your Healthcare The Community Dementia Service is a nurse led service for people with dementia and their families/carers that enables the person to maintain their independence and a live better for longer and is in keeping with the requirements of:
- NHS England Transformation Framework - The Well Pathway for Dementia (2016)
- World Health Organisation - Global Action Plan on the Public Health Response to Dementia 2017 - 2025
The Dementia Service works in partnership with the integrated health and social care teams and GP Practices in the Royal Borough of Kingston.
Successful candidate will carry out a range of duties under the direction of the Specialist Nurse Practitioner for Dementia and in accordance with related policies, procedures and guidelines. You will play an active role in reducing risks around developing dementia through health promotion.
- Assisting in all aspects of care for patients with dementia
- Assisting with daily living tasks
- Ensuring all clients are dealt with respectfully
You must be able to work independently without direct supervision, to ensure that the person with dementia and family/carer has access to research and evidence-based information and activity in their own homes or other care setting.
Requirements and prospects
- A good level of written and verbal communication skills
- Good knowledge of IT software to include Word and Excel
- A self-starter who is keen to learn
- The ability to work as part of a team in a professional environment
Applicants need to hold Level 2 Adult Care Worker Qualification or equivalent.
4 x GCSE A*-D/9-3 including maths and English or equivalent or relevant work experience and required initial assessment results.
Upon completion the apprentice will be encouraged to further progress their career within the organisation.
Things to consider
Candidates are required to have an enhanced DBS and provide the result prior to starting.
Ideally need to be 18+ with a driving licence and a car (preferred but not essential).
