Your Role at Your Healthcare Your Healthcare Community Interest Company’s Reablement service provides a Domiciliary Home Care and Reablement Service. The service provided short term planned, intensive intervention to maximise independence following a period of illness, disability or loss of self-confidence. This is an exciting opportunity for a highly motivated Adult Social Care Apprentice level 3 to join our team at Surbiton Health Centre.

You will provide reablement and rehabilitation support as the service user develops their own independence. This includes giving guidance and direction to service users and their carers in respect of daily living tasks, for example, dressing and personal care, meal preparation and practical support in accordance with the support plan. You will provide intensive personal, practical, social and emotional support by promoting independence and choice to people with disabilities, older people or ill people.

This will involve helping them to work towards their outcome-focused reablement and rehabilitation goals as set out in their support plan. In addition to meeting the person’s needs this role will:

  • Reduce the need for hospital or residential admission
  • Facilitate a safe and timely discharge
  • Provide active recovery and reablement/rehabilitation, thus preventing unnecessary loss of independence

As a community service the post holder needs to be able to travel between different sites therefore a full driving licence and use of a vehicle for work.


Requirements and prospects

Desired qualifications

  • Applicants need to hold Level 2 Adult Care Worker Qualification or equivalent
  • 4 x GCSE A*-D/ 9-3 including Maths and English or equivalent or relevant work experience and required initial assessment results

Future prospects

Upon completion the apprentice will be encouraged to further progress their career within the organisation.

Candidates are required to have an enhanced DBS and provide the result prior to starting.

Attendance to college once a week on day release toward the achievement of the Adult Care Worker Standard apprenticeship qualifications.

Drivers preferred but not required.
