To provide support to Heathcotes Group Service Users in line with the organisation’s aim to provide a high quality support service tailored to suit each individual’s person centred care plan and to promote independence and choice to maximise their potential.

Intermediate Level Apprenticeship
- Provide care and support as identified in care plans, as directed by senior staff members.
- To promote independence, life skills and informed choices in accordance with the individuals care plan and to enhance inclusion in the community.
- To provide personal care where required which may involve supporting with continence needs, health and medication needs, handling personal possessions, finances and documents, and shopping.
- To promote dignity and facilitate to ensure personal care provision is in a safe and appropriate environment, where directed through the care plan.
- To ensure effective communication and relationships with the service users and to be aware of the communication differences and adopt an approach that minimises the effect of such differences in accordance with their care plan.
- To manage behaviours adhering to strategies and procedures set out within individuals care plan and in accordance with relevant legislation.
- Ensure service users' individual needs are met through effective care planning and review. Review and monitor care plans.
- Interact and participate with the service users in the development of self-help skills, supporting individual’s to reach their potential for independence.
- Maintain professional knowledge and competence.
- Ensure you read your employee handbook thoroughly and ensure you read a copy of the Skills for Care Code of Conduct.
- To undertake any training required in order to perform the role more effectively, acknowledging any deficits in knowledge or practice, identifying training needs and formulating training plans.
- Act as a Keyworker to an individual service user, taking on responsibility for specified areas of care and support. Ensure all medical needs are met and support the individual to advocate his/her own needs and wishes.
- Work within the philosophies of the service to minimise the effects of service users’ learning disabilities and/or mental health, demonstrating an understanding and commitment to the rights of the individuals.
- Consistently apply non-aversive strategies developed in response to challenging behaviours, and contribute to the review and development of any such strategies.
- Encourage and enable service users to maintain acceptable levels of personal and environmental hygiene.
- Ensure that service users' personal property and clothing is maintained to a high standard, reporting any requirements to senior staff members.
- Practice maximum integrity in all dealings with service users' personal and financial affairs, and avoid abuse of the privileged relationship that exists with individuals.
- To contribute towards the protection of individuals from abuse, adhering to the organisational and local authority safeguarding policies and national legislation.
Requirements and prospects
- A good level of literacy.
- Good numerical skills.
- An awareness of the legislation surrounding the White Paper, Valuing People, Human Rights Act and Equal Opportunities and the Care Act.
- A good knowledge base of the needs of people with a disability.
- Proven ability to work on own initiative and as part of a team.
- A commitment to a service which provides support to vulnerable individuals, and which is respectful and according to the individuals’ wishes.
- Good communication skills (verbal and written). Ability to communicate effectively with service users, colleagues, managers and other persons with a legitimate interest in the service.
- Commitment to training and development.
- Caring/Support experience of working within a health and social care setting.
- General awareness of regulatory requirements as required under CQC.
- Knowledge and understanding of issues relating to people with learning disabilities and/or mental health.
- Flexible and innovative approach to working.
- Knowledge of care planning systems.
- Decision-making and problem-solving skills.
- Ability to communicate effectively within the written and oral form to develop and maintain constructive relationships with colleagues, families and professionals.
- Ability to demonstrate creativity and original thinking, whilst developing practical solutions to problems.
- Motivated to work towards creating a safe, open and trusting environment.
- Awareness of the issues around working with vulnerable adults and the boundaries of professional relationships.
- Good interpersonal skills and an ability to form positive relationships with colleagues.
- Good attitude towards being flexible, dependable, confident, friendly and a good sense of humour.
- The ability to be creative with behavioural distractions and activity participation in house and in the community.
- Be able to effectively encourage individuals whom refuse to participate in daily living skills, personal care tasks or activities.
- A willingness to work towards and achieve a minimum of QCF level 2 in Health & Social care within a given timescale.
- A willingness to undertake appropriate training.
